If injured, please follow the steps below:
- If it is a serious injury please call 911.
. - If the injury requires you to be seen by a doctor, it’s recommended to see one of the providers listed in the Liberty Mutual certified medical circle closest to your location. After navigating to that page:
- Choose the I am a customer option.
- Then select the Medical Provider Panel.
- When you fill in the address and click continue, a prefilled panel is created.
- Choose Create Medical Provider Panel to print or save a final copy.

- The next step is to fill out your work Injury Claim Form.
- Upon filling out this form and once it is processed by Potomac’s Human Resources team, the injured employee and the person they list as their supervisor will receive a claim number for the incident

- Follow your claim by:
- Visiting the Liberty Mutual website.
- Select Enroll Now button
- Enter your claim number and other required fields for authentication.
- Next enter required fields on registration form (note: your email address will become your username).
